Kansas requires school sports physicals before students can even start practicing this fall.
Register now for a convenient sports physical at one of our upcoming events.
IMPORTANT INFORMATION
• Athletes must wear athletic attire. No boots, flip flops, or jeans.
• Athletes 8th grade or younger must be accompanied by a parent.
• KSHSAA physical form must be filled out by
PARENT and ATHLETE prior to arriving.
• Bring athlete's vaccination record (optional)
• $25 per student, credit card, cash or check only.
